FINANCIAL POLICY

We are committed to building a successful relationship with you and your family. You must understand our Financial Policy as part of our professional relationship. Please remember that member services are a crucial aspect of this relationship.

 

We accept both insured and uninsured (cash-paying) patients. If you have any questions about our fees, policies, or your responsibilities, please do not hesitate to ask. Additionally, it is your responsibility to notify our office of any changes in patient information, such as address, name, or insurance details.​​​

 

 

Co-Pay

Patients are expected to present their insurance card at each visit. All co-payments and past-due balances must be settled at the time of check-in unless prior arrangements have been made with a billing coordinator. We accept cash, checks, or credit cards. Please note that we do not accept post-dated checks.​​

 
 
 
Insurance Claims

Insurance is a contract between you and your insurance company. In most cases, we are not a party to this contract. We will bill your primary insurance company as a courtesy, but to do so correctly, you must provide complete insurance information, including both primary and secondary coverage, as well as any changes to your insurance details. If you fail to provide this information, you may be responsible for the entire bill. While we may provide an estimate of what your insurance company may cover, the final determination of your eligibility and benefits rests with the insurance company. If our practice is not contracted with your insurance provider, you will agree to pay any charges not covered by your insurance, including those exceeding the allowable amounts. If we are out-of-network and your insurance pays you directly, you are responsible for forwarding that payment to us promptly.

If your insurance plan is not listed above, you will be responsible for payment in full. As a courtesy, we will file your initial insurance claim, but if payment is not received within 30 days, you will be responsible for the balance.

 
Referrals and Preauthorizations

Certain health insurance plans (HMO, POS, etc.) require that you obtain a referral or prior authorization from your Primary Care Provider (PCP) before seeing a specialist. If your insurance requires a referral or preauthorization, you are responsible for obtaining it. Failure to do so may result in a lower payment or non-payment from your insurance company, and you will be liable for the remaining balance. If you do not obtain the necessary referral or preauthorization, alternative payment arrangements or rescheduling of your appointment may be necessary.​

 
 
 
Self-Pay Accounts

Self-pay accounts refer to patients without insurance coverage, patients whose insurance plans do not include our practice or those who do not have an insurance card on file with us. Liability and workers' compensation cases will also be considered self-pay accounts.

  • Cerumen Impaction (one ear): $15.00  

  • Cerumen Impaction (bilateral): $48.00  

  • Dermabond: $120.50  

  • Simple Laceration (< 7.5 cm): $115.50  

  • Abscess Drainage (simple): $124.50  

  • Joint Injection (Large Joint): $65.50  

  • Joint Injection (Small Joint): $53.50  

  • Trigger Injection: $53.50  

  • Foreign Body Removal (ear/nose): $80.50
